Manulife Bank Financial Wellness Survey Contest
OFFICIAL RULES & REGULATIONS - Canada (excluding Quebec)
1. ELIGIBILITY
To enter and be eligible to win, you must be a resident of Canada who has reached the age of majority in your province or territory of residence as of November 1st, 2019. You are not eligible to enter this contest if you, your spouse, child or parent or a person you are domiciled with is an employee of Manulife Bank or an affiliate of Manulife Bank or their promotional or advertising agencies. Any decision made by Manulife Bank in respect of any entrant’s eligibility is final and non-appealable.
Quebec residents are not eligible for the Contest.
2. PROMOTIONAL PERIOD
Contest entries will be accepted from November 1st, 2019, at 12:01 am Eastern Time (ET), to November 17th, 2019, at 11:59 pm ET (the "Promotional Period"). To be eligible to win, your completed survey must be submitted online and received by no later than 11:59 pm ET November 17th, 2019, (the “Contest Closing Date”).
3. ENTRY
No purchase is necessary to enter or win. Only one entry per person is permitted. Each entrant will be eligible to win only one prize. All eligible individuals who participate in this survey and provide their contact information on their electronic ballot form by the Contest Closing Date will be entered into the Contest. To get access to the electronic ballot form, you must complete the Manulife Bank Financial Wellness Survey. Once the electronic ballot form has been completed, participants simply need to click on the "Submit" button and the form will be sent to Manulife Bank and entered in the Contest, if eligible. Online access is required in order to participate in this Contest.
4. DRAW
At 10:00 am Eastern Time (ET), November 29th, 2019, in Waterloo, Ontario, the potential winner will be randomly drawn from all eligible entries received by the Contest Closing Date. If, after making such efforts as Manulife Bank in its sole discretion deems reasonable, a potential winner cannot be contacted by email at the email provided on the contest entry within ten (10) business days of the official draw date, an alternate potential winner will be drawn. Nine (9) prizes will be awarded for the entire contest. The awarding of the prize is contingent upon the entrant's full compliance with these Contest Rules.
Once contacted, the potential winner will be required to correctly answer, unaided, a time-limited mathematical skill-testing question to be administered by Manulife Bank. All decisions of Manulife Bank in administering the skill-testing question, or his or her designate, shall be final. If the skill-testing question is not answered correctly, a new potential winner will be drawn from the remaining eligible contest entries. This process shall repeat until a potential winner, who has complied with these Contest Rules, is successfully contacted and correctly answers the skill-testing question and is thereby declared the contest winner (the “Prize Winner”).
5. PRIZE & ODDS OF WINNING
One (1) grand prize of $1,000 Amazon gift certificate and eight (8) $500 GoodLife certificates redeemable for personal training at any GoodLife fitness club in Canada (“the Prize”), will be awarded to the Prize Winners. The first randomly selected potential winner who answers the skill-testing question correctly will qualify to win the Prize, provided they comply with these Contest Rules.
The Prize must be accepted as awarded. Manulife Bank reserves the right to substitute a prize of equal or greater value if the advertised Prize is unavailable.
Odds of winning depend on the total number of eligible entries received for the drawing.
6. GENERAL CONDITIONS
a) Manulife Bank, the Contest Sponsor, reserves the right to terminate or alter this contest at any time or to alter the Contest Rules without prior notice.
b) The declared winner agrees to sign Manulife Bank’s standard declaration and release form confirming compliance with the rules and regulations and waiving any liability of Manulife Bank, its directors, officers, employees, and agents, affiliates and advisors with regard to injury, accident, or loss or damage of any kind arising out of participation in this promotion or acceptance of the Prize as awarded. If a declared winner is determined to be ineligible by Manulife Bank, or fails to return the completed declaration and releases as required within 10 business days of notification, Manulife Bank may declare the Prize to be forfeited and new potential winners may be selected and contacted, in accordance with rule #4 above, until a new winner is declared.
c) Manulife Bank assumes no responsibility for entries that are lost, stolen, delayed, damaged or misdirected either prior to or after delivery of the entry to Manulife Bank. Entries are subject to verification and will be declared invalid if they are or become illegible, mechanically reproduced, mutilated, forged, falsified, altered or tampered with in any way. Entrants agree to abide by these contest rules and the decisions of Manulife Bank, which are final. All entries become the property of Manulife Bank and will not be returned.
d) By accepting the Prize, the winner consents to the use of their name, address and/or photographs, without further compensation, in any advertising or publicity as may be carried out by Manulife Bank in its sole discretion.
e) If Manulife Bank determines that potential entrants are experiencing difficulty or are prevented from entering the contest due to technical difficulties, Manulife Bank may, without notice to any entrant or potential entrant, extend the contest for such period of time as Manulife Bank deems appropriate in the circumstances. Request for the name(s) of winner(s) must be received within 90 business days of the Contest Closing Date.
